You’re invited to participate in our Provider Advisory Survey. We want to collaborate with you to find ways to enhance our partnership with our providers.
How it works: We’ll email this survey to randomly selected providers in early September. If selected, you may receive the survey as soon as Monday, Sept. 9. If you do, please complete the simple five-question survey at the web address provided on the survey.
Your feedback helps us better serve both you and our members. If you are selected and complete the survey, you will be automatically entered in a drawing to win one of five $100 gift cards, courtesy of BCBSOK.
Survey deadline: If you receive a survey, please complete and return it no later than Friday, Sept. 27.
Who may take the survey: A physician, nurse, office manager or other qualifying staff may complete the survey.
